Cornerstone alternatives: a practical shortlist
Cornerstone's strength is IDEXX integration and its reported weaknesses are learning curve and cost. Leaving is a bigger project than most, which is why it is worth planning.
The short version
The obstacle is rarely the new system. It is years of workflow nobody has written down, and the equipment integration you would be unpicking.
Best for: Established Cornerstone practices weighing a move to cloud.
Strengths
- Tight integration with IDEXX in-house lab and imaging
- Very large installed base, so staff often arrive trained
- Works without an internet connection
Trade-offs
- Commonly reported as difficult to learn, with an interface many dislike
- Software, support and separately-charged hardware support reported as expensive
- Integrating with non-IDEXX software reported as time-intensive
Cornerstone is one of the most widely deployed practice management systems in US veterinary medicine. Reviewers are consistent about both sides of it: the integration with IDEXX diagnostics is the reason to stay, and the learning curve, the interface and the cost are the reasons people look elsewhere. Hardware support is charged separately, which reviewers raise repeatedly.
Start by writing down what you actually do
Before looking at alternatives, spend a week documenting your real workflow, including the workarounds. A practice running Cornerstone for a decade has built report templates, invoice structures, reminder logic and equipment integrations around it. None of that is in a vendor's migration scope, and all of it has to be rebuilt somewhere. That document becomes your evaluation criteria.
The equipment question comes first
This decides more than anything else. Reviewers report that integrating Cornerstone with non-IDEXX software is time-intensive, which is the flip side of how well it integrates within the IDEXX estate. So:
- If you are staying with IDEXX equipment, ezyVet keeps you inside the same ecosystem while moving you to cloud. It rates 4.6 across 276 Capterra reviews, the highest we cover. IDEXX's Neo is the lighter option for smaller practices.
- If you are not, or you want the decisions separated, then Provet Cloud, Vetspire and Woovet are all outside the IDEXX estate. Woovet is not owned by a diagnostics or distribution group at all.
Practices often discover the software and equipment decisions are more entangled than either vendor leads with. Our guide to who owns your practice management system sets out who owns what.
The multi-site question
If you are running or planning more than one location, this usually settles matters on its own. Server-based systems make multi-site possible but awkward. Cloud systems treat it as the normal case.
What actually moves
The part practices underestimate is the data, not the software. Structured records (clients, patients, appointments, invoices, reminders) transfer well. Clinical notes usually transfer but can arrive flattened, with SOAP structure collapsed into plain text. Attachments are the most variable: files rarely vanish, but they can arrive without the link to the right patient.
Ask any vendor for a test migration against a copy of your real database before you sign. What comes back tells you more than every demo combined. We have written the full version of this in what actually transfers when you change practice software.
